To port an 8xx (toll-free) number to AVOXI, our supplier requires a Letter of Authority to be filled out.

Why This Is Required

The LOA must be dated within 30 days and needs a wet (physical) signature — a digital signature is not accepted.

What You Need to Provide

• The completed AVOXI Standard LOA Form - US/CAN Toll Free, dated within 30 days, with a wet signature
• If the numbers you're porting don't all fit on the standard form, use the AVOXI Multinumber LOA Form - US/CAN Toll Free instead — do not edit the standard form to add space

How to Submit It

• Download the appropriate LOA form (standard or multinumber) and complete it with a wet signature, dated within 30 days.
• Upload it in your AVOXI online account. See our informative guide for help.
