Must include the full Company Address
Must include the Swedish Organization Number (Org.nr)
Must include the name of the Current/Losing Carrier
Must include the list of Sweden Toll-Free numbers to be ported
Must include the printed name of the authorized signatory
Must include the title of the authorized signatory
Must be signed by the authorized signatory
Must include the date of signature
How to Submit It
Download and complete the AVOXI Letter of Authority.docx with all required company and signatory details, and the numbers being ported. Have a legal representative and an authorized contact on the losing carrier account sign and date it.
