Port Document Required - Portability Form (Peru DID)

 

To port your number, please have the authorized contact/legal representative the same listed in the Peru Power of attorney (Vigencia de poder - Validity of Power) complete the attached Peru Portability form with signature and fingerprint

 

The porting process could be delayed if the form is not completed by an authorized contact/legal representative listed in the Peru Power of attorney (Vigencia de poder - Validity of Power) and if we are missing both the Signature and the Fingerprint.

 

Once the form is hand-signed and you have added the fingerprint of the legal representative which needs to be the same listed in the Peru Power of attorney (Vigencia de poder - Validity of Power). Please proceed to share it.

AVOXI Core Platform

If you are an AVOXI Core customer you can track your order status with your Onboarding Specialist who will also provide regular email updates. Lead times on orders are based on inventory at the time of order.  Most numbers are usually ready the next day unless there is a long order time with the carrier. Contact your Account Manager about an AVOXI Platform Demo and Migration.
