To port your number, please have a legal representative complete the attached port form. The porting process could be delayed if the form is not completed by an authorized contact on the losing carrier account.
Below are the highlighted sections to fill out.
Section 1 - This is the Company Name, and Registration Number
Section 2 - List the numbers we are porting and the portability validation code (current supplier should be able to provide this information. If we are porting a range, please fill out the range instead of listing the numbers individually.
