Port Document Required - Letter of Authority - (Poland Local)

To port an existing Poland local number to AVOXI, our carrier requires a completed and signed port form from a legal representative on the losing carrier account.

Why This Is Required

Poland's carrier requires a signed Letter of Authority from an authorized contact on the current (losing) account before it will release the number for porting. This confirms you have the authority to move the number to AVOXI.

What You Need to Provide

  • The completed Poland Porting Form (Mandatory)  — must be filled out and submitted in the original Polish version

  • An English Translated Document fields  reference version of the form is available for guidance only — it is not an accepted submission on its own

  • Fields highlighted in yellow and blue must be filled out and selected

  • Must be completed by an authorized contact on the losing carrier account — an unauthorized signer can delay the port

How to Submit It

  1. Log in to the AVOXI Genius platform and go to the relevant number's profile page.

  2. Download the Poland Porting Form (Mandatory) (original Polish version — required for submission).

  3. Use the English-translated version only as a guide for which fields to complete.

  4. Have an authorized legal representative on the losing carrier account complete and sign the Polish-language form.

  5. Upload the completed form directly from the number profile page.

Note: The number will not work and remains blocked until the documentation is received and registered with the local provider.
