To port an Italy toll-free number to AVOXI, you need a signed port form.
Why This Is Required
A legal representative must complete this form before AVOXI's carrier will release your number for porting.

- Signature from a legal representative of your company

Important Notes:
- The porting process could be delayed if an authorized contact on the loosing carrier account does not complete to above form.
- If your number is currently under a reseller (not a local carrier from Italy), please include the Italy carrier that owns these numbers (also known as the upstream/local or Donor Operator), as this information is required to submit the request.
How to Submit It
- Download and complete the port form. Have a legal representative sign it.
- Upload the signed form in your AVOXI online account. See our informative guide for help.
- Our number team will review it. Your account status updates automatically once we finish.
