Proof of number of insured employees , based on Labor insurance certificate, dated within the past one month
Understand that you must provide the information regarding the number of insured units from the Bureau of Labor Insurance, and apply through the e-service system of the Bureau of Labor Insurance - Simply providing the number of people is not sufficient
