When porting a Switzerland Toll Free number to AVOXI, our carrier requires the attached port form to be filled out and signed.
Please be sure to have the POA filled in directly by the owner or have a power of attorney signed by the owner of the number.

Important Note: Each Switzerland Toll-Free number has a registered user/owner on file with the Federal Office of Communications (OFCOM/BAKOM), which oversees media and telecommunications in Switzerland.
Please ensure that, before completing the LOA, you are the owner of record. If not, the LOA must be completed by the actual registered owner.
Kindly contact your current carrier, as they are the only party that can disclose the registered owner information, update the owner of record, or provide the necessary authorization.
