IMPORTANT NOTE - This is mandatory: 

1. Completed and stamped Application Form (with both your Taiwanese company seal and the legal representative’s seal of your Taiwanese company) — (these are two different seals, make sure to add both to your form)
2. The Declaration Form includes a list of numbers to describe the customer's actual business type, a list of applied numbers, their main uses, and call content.
   1. For example, if the customer is actually promoting ABC English online courses, the purpose cannot simply be written as "[Making promotional calls]". The customer should provide a detailed description, writing "[Promoting ABC English online courses]"
3. **Numbers not listed in the number list or without the company seal cannot be activated by our company.

All forms must be completed in Chinese (except for foreign names)
